I, the undersigned, legal representative of the child named in this form, attest to the accuracy of this strictly personal and confidential information and undertake to inform Gemmez Inc. without delay if any change occurs to the information submitted above. I certify that the above-mentioned child is in good physical condition and that no doctor, nurse or other person has advised me against practicing gymnastics or other related physical activities. I certify that I have parental authority over the above-mentioned child.

Académie d'Excellence Gemmez Inc. may take photographs and/or video recordings during training, events, and competitions for promotional use on our website, social media platforms, and printed materials. Your child may be identifiable in these materials.

You may withdraw this consent at any time by sending a written request to admin@gemmez.com.

In the event of an emergency, should it be necessary to take your child to the hospital or doctor, we undertake to contact the parent(s) as soon as possible, if your child is under 18. If we are unable to reach the parent(s), the undersigned authorizes us to provide emergency medical care for the child, as prescribed by the physician. I also authorize them, in the event of an accident, to call for help or to transport my child to a hospital, all at my own expense if necessary. Ambulance costs, if any, will be the responsibility of the child's parents.

As parent and/or guardian of the named child, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child, in the event of a medical emergency which, in the opinion of the attending medical professional, requires immediate attention in order to prevent further endangerment of the minor's life, physical disfigurement, physical disability or other undue pain, suffering or discomfort, if delayed.

Authorization is granted to the attending physician to carry out any minor medical or surgical treatment, radiological examination for the named child. In the event of an emergency resulting from serious illness, the need for major surgery or significant accidental injury, I understand that the attending physician will make every effort to contact me as soon as possible. This authorization is granted only after reasonable efforts have been made to contact me.

Authorization is also granted to Académie d’Excellence Gemmez Inc. and its affiliates, including team directors, coaches and parents, to provide necessary emergency treatment prior to the child's admission to the medical facility. This authorization is given of my own free will, for the sole purpose of authorizing medical treatment in emergency circumstances, for the protection of the life and physical integrity of the designated minor child, in my absence.
